Wilkes was a member of parliament for Middlesex, and a radical journalist who edited the anti-government newspaper The North Briton. The medal commemorates the publication of an edition of The North Briton where Wilkes criticised the power of the monarchy.

The face of the medal shows Wilkes in profile, with his name around the edge.
The reverse shows Old Father Time as a figure with wings holding a sythe. He is engraving a stone with '45 North Briton' above a base which says 'Magna Carta'. Below the illustration is engraved 'In memory of the year MDCCLXVIII' (1768)
